Emergency Medical

Accidents happen and they can occur at any time. If you have any allergies, have any medical conditions, or take any medication that might be important for a first responder to know in the event of an accident please fill out this form. Drop it off with Robert Paige on race day. He'll be located near the slide at the top of the hill. Your information will be treated confidentially and all forms will be destroyed immediately following the race.

In an emergency situation, clarity is important.
-Make sure to circle which race you are in and enter your race number. That will be the quickest way to access your information.
-He doesn't need to know you complete medical history, only what would be critical if you were unconscious or unresponsive, what you would include on a medical alert bracelet for example. In the same vein, please either type or print legibly.
